FAMILY SUPPORT SERVICES
FAMILY SUPPORT SERVICES
Family support services aim to provide the family members of the child accessing THRIVE services the support, resources, and education that they need.
SOCIAL WORK
Our social work team works closely with the family and the child to provide holistic support, advocacy, and empowerment. This support may look different depending on the needs of the family. The Social Worker will work closely with the family to assess and identify how we can best support.
- Services we provide include:
- Supporting families whose child is newly diagnosed
- Emotional support with coping related to diagnosis
- Transition to JK or high school (when ICDP, ELR, or CLA is not involved)
- Funding support (i.e., diagnosis specific, equipment, etc.)
- Service navigation, referrals, and case management
- Parenting support and strategies in relation to developmental challenges
COORDINATED SERVICE PLANNING
Coordinated Service Planning (CSP) is a provincial initiative to help families with children with complex needs. The goal of CSP is to assist families to coordinate their child's services, be the lead or go-to for the family, and oversee a service plan based on the family's goals. The outcome of this is that families will have ONE coordinator, FEWER meetings and having to repeat their story, and ONE plan that keeps everyone on their child's team connected and working on family-driven goals.
If you think CSP is a good fit for your family you can self-refer at THRIVE and we will match you with a Service Planning Coordinator. Call 705-759-1131, Ext. 2.
INFANT AND CHILD DEVELOPMENT PROGRAM
The Infant and Child Development Program is a home-based program that provides services to children from birth to school entry who have an established developmental delay or are identified as at risk for delay. Families are provided a variety of services based on the individual child and family’s needs.
These include:
- Regular home visits by an Infant and Child Development Consultant
- Monitoring of the child’s development through developmental screening and assessment
- Play-based developmental programming and activities to meet the child’s needs
- Information and support to families about their child’s development or diagnosis
- Assistance to families in accessing appropriate community resources
Referrals to the Infant and Child Development Program are accepted directly from parents, or from a physician, therapist or community service provider on their behalf. Please see the referrals page for ways to refer to the ICDP program
FETAL ALCOHOL SPECTRUM DISORDER
THRIVE has two FASD Workers to support children, youth, and their families with Fetal Alcohol Spectrum Disorder or suspected Fetal Alcohol Spectrum Disorder. They can work with the individual family and are involved in our community, promoting awareness, advocacy, and education. They are active members of Algoma's FASD Committee. FASD workers provide support to families who would benefit from in-home strategies, advocacy and support at school, complex case coordination, and service navigation or information on how to obtain a diagnosis.
